Dr. Cory Torgerson’s Approach to Addressing Native Hair Loss via Strategic Recipient Site Planning and Multimodality Therapy

TORONTO, Canada, Sep 14, 2026 — Dr. Cory Torgerson and The Toronto Hair Transplant Clinic have introduced a combined hair-restoration protocol integrating Follicular Unit Extraction (FUE hair transplant), platelet-rich plasma (PRP), and low-level laser phototherapy. The approach is designed to restore coverage in areas affected by hair loss while supporting the preservation of native hair surrounding the transplant recipient sites.

Hair transplantation relocates healthy grafts to areas experiencing hair loss, but native follicles that have not been transplanted may remain susceptible to androgenetic miniaturization. The clinic’s protocol addresses this longer-term concern by combining surgical restoration with nonsurgical therapies intended to support both transplanted grafts and existing follicles.

Planning Transplant Density Around Native Hair

Recipient-site planning requires graft density to be balanced against the patient’s anatomy, donor capacity, existing hair, and pattern of hair loss. Native density refers to the natural concentration of follicular units before miniaturization, while transplant density is generally measured by the number of grafts placed per square centimetre during restoration.

Because an individual graft may contain between one and four hairs, grafts per square centimetre and the number of visible hairs per square centimetre do not always provide the same assessment of density.

According to information published by the National Institutes of Health, androgenetic alopecia involves progressive follicular miniaturization in which terminal hairs gradually become thinner, shorter vellus-like hairs. When recipient sites are placed too closely during transplantation, blood flow within the dermis may be compromised, potentially affecting graft survival. Excessive density targets may also place unnecessary pressure on the available donor supply.

The clinic therefore plans density around the visual coverage that can be achieved without over-extracting the donor area or placing grafts too closely within recipient sites.

Anatomical Factors Affecting Density Planning

The density required to produce natural-looking coverage varies according to the physical characteristics of the patient’s hair and scalp.

Hair calibre affects how much visual coverage each strand provides. Thicker shafts create greater visual shadow across the scalp and may produce the appearance of density with fewer grafts, while finer hair may require a different density objective.

Curl and wave patterns also influence coverage. Curly or wavy hair tends to extend outward and overlap adjacent strands, which may create greater visual density than straight hair at a comparable number of grafts per square centimetre.

The contrast between hair and scalp colour is another consideration. Lower contrast may make the scalp less visible, while higher contrast—such as dark hair against lighter skin—can make scalp exposure more noticeable under bright lighting. This requires particularly careful planning along the hairline and transition zones.

Combined FUE, PRP, and Laser Phototherapy

The Toronto Hair Transplant Clinic’s protocol combines three treatment components with distinct roles in hair restoration and native-hair maintenance.

FUE uses micro-punch extraction and graft-placement techniques to relocate follicular units to depleted areas of the scalp, including the hairline, mid-scalp, and crown. Its primary role is to restore physical coverage where hair density has been lost.

PRP uses the patient’s own blood-derived platelet concentrate, prepared through centrifugation and administered through targeted injections. The treatment is intended to deliver growth factors to the dermal papilla and surrounding microvascular networks, supporting compromised native follicles and the scalp environment around transplanted grafts.

Low-level laser phototherapy uses red-light treatment within the 650–670 nm wavelength range. The therapy is intended to support cellular activity through photobiomodulation and help manage inflammation within treated scalp tissue.

Peer-reviewed systematic reviews indexed in PubMed have examined autologous growth-factor treatments and photobiomodulation in relation to hair growth and follicular maintenance. Within the clinic’s protocol, these nonsurgical therapies complement transplantation by addressing native follicles that remain vulnerable to ongoing miniaturization.

Consultation and Individual Density Evaluation

Treatment planning begins with an assessment of the patient’s donor potential, recipient-site requirements, hair characteristics, and areas of active miniaturization.

During the consultation, the scalp may be examined under magnification to assess hair-shaft diameter, follicular groupings, and signs of miniaturization in native hair surrounding the proposed recipient sites. This evaluation helps distinguish stable hair from follicles that may remain at risk of further thinning.

The donor area is then mapped to determine the number and distribution of grafts that may be extracted without unnecessarily reducing donor density. Recipient requirements are evaluated by zone, with graft selection and placement adjusted across the hairline, mid-scalp, and crown.

Single-hair grafts may be used along the edge of the hairline to create a natural transition, while grafts containing multiple hairs may be positioned in areas where greater visual density is required. The resulting plan is intended to balance immediate restoration with the long-term preservation of both donor resources and native hair.
